Providence Healdsburg Hospital is seeking a part-time Patient Navigator – Case Management to join our care coordination team. The role supports a statewide MOUD access initiative by conducting initial assessments, connecting patients to MOUD programs and services, expediting clinic appointments, and coaching clients through their care plan.

The position requires relevant California licenses or license tracks, a social work degree, and experience with substance use disorders.

Qualifications

  • CA-licensed behavioral health professional upon hire.
  • Master's degree in Social Work preferred for this role.
  • Licensure/certification required upon hire.
  • One year of experience with Substance Use Disorders is desirable.

Responsibilities

  • Conduct initial assessments and identify MOUD needs.
  • Introduce patients to MOUD programs and services.
  • Expedite appointments at MOUD-capable clinics and coordinate care.
  • Maintain ongoing contact with clients and coach them through their plan.
  • Assist with access to financial counseling, primary care, mental health, social services, and residential treatment facilities.

Education

Bachelor's Degree in Social Work.
Master's Degree in Social Work.
California Licensed Behavioral Health Counselor or California Alcohol and Other Drug Counselor Certification.
California Clinical Social Worker License.
California Marriage and Family Therapist License.
1 year experience working with patients with Substance Use Disorders.

The Care Navigator is a care coordinator and an integral team member of a statewide initiative to improve access to Medication for Opioid Use Disorder. Navigators conduct initial brief assessments, introduce patients to MOUD programs and services, expedite appointments at MOUD-capable clinics, serve as the primary coach for their clients, and maintain ongoing contact with their panel. They also assist with access to other services such as financial counseling, primary care, mental health services, social services, and residential treatment facilities.

Preferred Qualifications

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Pay Range: $39.33 - $61.07. The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
