Job summary

Crossroads Treatment Centers is seeking a Part-Time Patient Navigator to assist patients at Belvidere and Stanhope, NJ. You will greet patients, explain treatment options, and connect them with the care team and resources.

You will coordinate appointments, participate in care planning, document recommendations, and monitor patients across the care continuum, including inpatient events. Training is provided, with emphasis on outreach and collaboration.

Qualifications

• Must hold RN, LPN, or BSW with a Bachelor's Degree and 2 years lived addiction experience.
• Alternatively, an Associate's degree or CMA with 4 years lived addiction experience.

Responsibilities

• Greet patients and answer clinic phones and assist with treatment options.
• Serve as a link between patients and the care team for appointments and services.
• Coordinate multidisciplinary planning conferences and document recommendations.
• Follow patients through the care continuum and support inpatient and outpatient care.
• Assist with outreach, data collection, and relationship-building with physicians and staff.
• Support stocking supplies and the Patient Ambassador program.
