Franklin Primary Health Center is seeking a Patient Navigator to participate in care management and coordination for primary care patients. You will develop workflows, support clinical teams, and use patient registries and payer portals to track progress in our EHR system.
Strong organizational skills and proficiency with Excel, Word, and Office Suite are essential. You will engage with patients and payers to ensure high-quality care and compliant documentation.

The Patient Navigator actively participates in care management and coordination activities for patients of the primary care practice. This includes developing and monitoring care coordination workflows, supporting clinical teams, and using patient care registries—both internal and payer-supplied—to track progress. The Patient Navigator utilizes applicable Third-Party Payer portals to transcribe and submit patient data as documented in Franklin’s EHR system. All activities are reported using a standardized process. Strong organizational skills, exceptional customer service, and proficiency in Microsoft Excel, Word, and Office Suite are essential to succeed in this role. The Patient Navigator is expected to consistently behave professionally and provide high-level support to both the patient panel and care teams.

Must adhere to safety regulations, personnel policies and procedures.

Knowledgeable of OSHA and CLIA requirements to assure compliance.
Maintain yearly health maintenance records by obtaining TB skin testing and completion of the hepatitis series.
Adheres to Health Insurance Portability and Accountability Act (HIPAA Privacy Rule) policies and procedures.
Must successfully complete Privacy Rule compliance training annually or as revisions are made to the policies and procedures.
Must comply with Privacy Rule guidelines by learning to protect FPHC patient’s medical privacy.
Must comply with Privacy Rule guidelines by appropriately maintaining our patient information in compliance with national standards.
Must comply with Privacy Rule guidelines by providing appropriate security of FPHC patient records.
