Community Health Network seeks a Patient Navigator to provide individualized assistance to patients, families, and caregivers, helping overcome barriers and ensuring timely access to quality medical and psychosocial care.
You will collaborate with the care team to implement the patient's plan of care, support oncology survivorship navigation, and help patients understand their diagnosis and care pathways. Roles include coordinating with South and North Cancer centers, with one WFH option day.

The Patient Navigator offers individualized assistance to patients, families and caregivers to help overcome health care system barriers and facilitate timely access to quality medical and psychosocial care. They collaborate with other members of the care team to provide consistent access to exceptional care delivery. The navigator will participate in development of the patients’ plan of care and implementation of the plan.
Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving.
This position will support our South Cancer center 2 days per week, North Cancer center 2 days per week with one WFH option day. This position will support our oncology survivorship navigation program.
Certified Medical Assistants, Social Workers (BSW), Licensed Practical Nurses, or Certified Community Health Workers also eligible with 2+ years of oncology experience
