Job summary

Miami Jewish Health in Fort Lauderdale, FL is seeking a Patient Coordinator to manage clerical tasks, appointments, and data reporting for PACE participants. The role interfaces with caregivers, ALFs, escorts and transportation to ensure timely patient care.

The position requires a high school diploma and 2 years of healthcare-related clerical experience, with bilingual Spanish/English preferred and knowledge of medical terminology. On-site, full-time opportunity with benefits.

  • Coordinates with caregivers, transportation, ALF staff and escorts to ensure attendance.
  • Oversees unit-specific documentation per department guidelines and regulatory standards.
  • Provides coordination with other departments and supports reception.
  • Attends department meetings and trainings as needed.

Brief Description Miami Jewish Health is one of the largest providers of healthcare and living options for aging adults in the Southeast. We operate Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, the largest PACE program in Florida, which serves as health plan, healthcare provider, and social center designed for older adults with complex medical needs. We have four adult day health centers that serve participants in Miami-Dade and Broward counties.

Job Title: Patient Coordinator

Job Summary

Perform clerical duties utilizing specific knowledge of medical terminology, PACE regulations, contract providers and authorization procedures. Duties include scheduling appointments for participants, including coordination with caregivers, ALF’s, escorts and transportation. Pull reports from information system to monitor compliance. Oversee scanning of assessments by medical receptionist into software system.

Essential Job Functions
  • Schedules all participant diagnostic appointments and medical consultation for PACE participants and enters appointments into PACE medical authorization system.
  • Tracks appointments and produces reports as requested in order to track data.
  • Coordinates with caregivers, transportation department, ALF staff and escort, if needed, in order to assure that participant attends the appointment.
  • Oversees unit specific documentation as per department guidelines and regulatory standards.
  • Provides necessary coordination with other departments
  • Acts as support for receptionist
  • Performs other support functions as needed.
  • Attends department meetings and in-services as needed.
Education
  • High School Diploma or equivalent
Experience
  • Two years office/clerical experience preferably in healthcare setting
