Job summary

CivicMinds, Inc. in Palm Springs, CA is seeking an experienced Nurse Practitioner or Physician Assistant to deliver high-quality, patient-centered care in a managed care setting.

You will conduct comprehensive assessments, diagnose and manage conditions, develop treatment plans, order tests, coordinate referrals, document encounters in EHR, and collaborate with physicians and staff to ensure ongoing, standards-based care.

Qualifications

• Active California NP or PA license.
• Experience in Gastroenterology, Internal Medicine, Primary Care, or related specialty.
• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current BLS certification.
• DEA registration or eligibility.

Responsibilities

• Conduct comprehensive patient assessments, including medical histories and physical examinations.
• Diagnose and manage acute and chronic medical conditions under applicable state regulations and clinical protocols.
• Develop and implement treatment plans in collaboration with supervising physicians.
• Order, perform, and interpret diagnostic tests, laboratory studies, and imaging results.
• Prescribe medications and therapies as permitted by state law and organizational policies.
• Provide preventive healthcare services, patient education, and counseling on disease management and healthy lifestyle practices.
• Assist with medical procedures within the scope of practice and demonstrated competency.
• Coordinate referrals and follow-up care with specialists and ancillary healthcare providers.
• Document patient encounters accurately and timely in the electronic health record (EHR) system.
• Collaborate with physicians, nurses, and other clinical staff to ensure continuity and quality of care.
• Participate in quality improvement initiatives and maintain compliance with clinical and regulatory standards.
