• Performs administrative duties including scheduling, data entry, and patient registration.
• Reviews and documents Social Determinants.
• Provides education and resources as needed.
• Monitors and communicates changes in patient condition.
• Documents patient care provided.
• Provides discharge instructions and follow-up with patient.
• Maintains equipment and medical supplies, including stocking and cleaning/disinfection.
• Rounds with patients to ensure Excellent Patient Experience.
