Job summary

LiveWell Homecare Agency is seeking a full-time nurse to provide direct patient care, assessments, and care planning in a Missouri setting. The role emphasizes patient-centered care, education for patients and families, and collaboration with the interprofessional team to develop and communicate individualized care plans.

Qualifications

• RN license required; Nursing Diploma or Associate’s Degree acceptable.
• Bachelor’s Degree in Nursing and at least 2 years of experience preferred.
• Ability to work in a safety-sensitive environment with alertness and safety awareness.

Responsibilities

• Deliver patient and family-centered care, including education on post-treatment care and medication management
• Participate in quality improvement, safety initiatives, and professional development
• Develop and document individualized care plans with clear goals, ensuring effective communication across care teams
• Assess and address physical, psychosocial, emotional, spiritual, cultural, and population-specific patient needs
