Job summary

Avera Creighton Hospital in Sioux Center is seeking an LPN to join a multidisciplinary team. The position features rotating shifts from 7pm-7am and 7am-7pm, averaging 72 hours per two weeks with every third weekend/holiday rotation.

The LPN earns a competitive pay scale across the PCT and LPN ranges and may qualify for a $15,000 sign-on bonus. Under supervision, the LPN coordinates care, conducts focused assessments, and helps develop patient-centered plans.

Qualifications

Position Highlights

PCT Pay scale: $18.00 - $20.75 - $23.50

LPN pay scale: $24.50 - 29.13 -33.75

A Brief Overview

The LPN practices under the supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which he/she is accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members.

What You Will Do

• Collects data and conducts a focused nursing assessment of the health status of a patient.
• Participates with other health care providers in the development and modification of the patient-centered health care plan.
• Implements nursing interventions within a patient-centered health care plan.
• Assists in the evaluation of responses to interventions.
• Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
• Advocates for the best interest of the patient.
• Communicates and collaborates with patients and members of the health care team.
• Assists with health counseling and teaching.
• Delegates and assigns nursing interventions to implement the plan of care.
