Job summary

Lehigh Valley Health Network is seeking an entry-level patient care teammate to support rehabilitation services on site in Hazleton. You will assist with daily activities, prepare patients for therapy, and work closely with licensed clinicians to ensure safe, high-quality care.

The role emphasizes flexible scheduling, observation of patient conditions, and effective communication with the care team to support patient well-being in a fast-paced hospital environment.

Qualifications

  • Minimum qualifications include a high school diploma or GED.
  • Ability to recognize and respond to changes in patient condition.
  • Competent in monitoring basic vital signs (HR, BP, RR).
  • Demonstrates BLS certification upon hire.
  • Flexibility to work outside normal hours to meet patient needs.

Responsibilities

  • Provides care-related activities for designated patients under supervision by RN/PT/OT/ST/therapist staff.
  • Reports observations/changes in patient status to nursing/medical personnel and performs hourly rounds.
  • Assists with transfer, gait, and daily living activities.
  • Prepares patients for therapy and may assist with transport to therapy sessions.
  • Ensures patient safety and follows care guidelines to reduce risk of harm.
  • Documents intake/output and operates safety equipment as required.
