Job summary

Assured Home Health, part of the LHC Group and Optum family, is seeking a Patient Care Manager Assistant in Medford, Oregon. The role supports the Patient Care Manager and coordinates a team to maintain high-quality home health care for patients in the assigned area.

The ideal candidate is an LPN with an unrestricted license in Oregon, has a valid driver's license, and access to reliable transportation. CPR certification is preferred, and home care experience is a plus.

Qualifications

• Current unrestricted LPN license in Oregon.
• Valid driver's license and access to dependable transportation.
• CPR certification preferred or able to obtain within 90 days of hire.

Responsibilities

• Assist the Patient Care Manager in coordinating a team to ensure high-quality care for patients in the assigned area.
• Prepare patient lists and documents for weekly case conferences.
• Provide direct patient care as needed per scope of practice, plan of care and physician orders, coordinating with the health care team.
• Assist with clinician documentation reviews/audits to ensure complete, accurate submissions by field staff.

As the Patient Care Manager Assistant, you are responsible for assisting the Patient Care Manager in coordinating a team of staff to assure the continuity of high-quality care to patients assigned to the team's area.

Primary Responsibilities

• Ensures that the team is committed to providing quality care in an efficient manner in accordance with physician's orders and State/Medicare guidelines
• Prepares patient list and necessary documents for weekly case conferences
• Provides direct patient care, as necessary, in accordance to scope of practice, plan or care and physician orders, and in coordination with other members of the health care team. May not alter the plan of care
• Assist Patient Care Manager in clinician documentation review/audits to ensure complete, appropriate, and accurate submission of all documentation by field staff
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications

• Current unrestricted LPN licensure in state of Oregon
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire
• Home care experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.70 to $43.05 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
