Description

The Patient Care Coordinator serves as a liaison between Tribal Citizens and healthcare providers, community resources, and supportive services. This position assists Tribal Citizens in accessing medical, behavioral health, social service, and wellness resources across a multi-county service area. The Patient Care Coordinator focuses on care navigation, advocacy, referral coordination, outreach, education, and helping Citizens overcome barriers to care. The Patient Care Coordinator promotes culturally responsive services and supports the Tribe’s commitment to improving the overall health and wellness of Tribal Citizens.

  • Coordinates referrals and follow-up care with outside healthcare providers, hospitals, pharmacies, and community agencies.
  • Helps Citizens navigate Medicaid, Medicare, private insurance, and other healthcare coverage programs.
  • Assists with enrollment, renewals, prior authorizations, and healthcare-related paperwork.
  • Provides care coordination and support for Citizens with chronic health conditions, behavioral health needs, substance use disorders, disabilities, or complex care needs.
  • Maintains regular communication with Citizens to ensure continuity of care and follow-up services.
  • Advocates for Citizens experiencing barriers to healthcare access, transportation, housing instability, or social service needs.
  • Collaborates with Tribal departments and community partners to support whole-person wellness initiatives.
  • Maintains accurate and confidential client records in compliance with HIPAA and Tribal policies.
  • Assists with outreach events, wellness fairs, health education activities, and community engagement efforts.
  • Supports Citizens through compassionate, trauma-informed, and culturally appropriate interactions.
  • Provides resource information and referrals related to food assistance, transportation, elder services, disability support, and other community programs.
  • Participates in required trainings, staff meetings, and professional development opportunities.
  • Performs other duties as assigned.
Required Qualifications

Preferred Qualifications
  • Associate’s degree in Human Services, Healthcare Administration, Social Work, Behavioral Health, or related field.
  • Two years of experience in healthcare coordination, social services, case management, behavioral health, Tribal health, or related field.
  • Knowledge of Medicaid, Medicare, healthcare systems, and community resources.
  • Experience working with Native American/Alaska Native communities preferred.
  • Experience with trauma-informed care and culturally responsive services.
  • Knowledge of Tribal healthcare systems and urban Indian health resources.

Applicants must be a Washington State resident or willing to relocate to Washington State.

Working Conditions

Physical Demands: Long periods of sitting (60% of time); occasional bending, crouching, stooping, stretching, reaching, lifting, carrying, and pushing (20% of time); and occasional standing and walking (20% of time). Occasionally lifts/carries up to 20 lbs. Must possess manual dexterity sufficient to use office equipment with frequent repetitive motion such as writing, typing, paperwork-handling, and phone use. Must possess the ability to view print on a computer screen and/or paper document; the ability to speak and hear exchanging information with end users by telephone Mental/Visual Demands: Frequent mental and/or visual attention to task is required. Occasionally work requires heavy concentration. Frequent interruptions. Tasks involve eye-hand coordination. Working Environment: Work environment is mainly indoors with heat and some air conditioning. Must sit or stand at desk for the majority of shift. Occasionally works alone without benefit of supervisor or co-worker. Unavoidable hazards/risks: Work requires minimal risks which require safety precautions (using some cleaning chemicals for light cleaning). Protective clothing or equipment may be required (goggles, gloves, masks, boots, etc.)
