Description

Employed byPAGNYforThe Division of Correctional Health Services (CHS)of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city.

Patient Care Associates (PCAs) work under the direction of a licensed nurse or other medical staffto provide patients with care and support. PCAsperform venipuncture, finger-sticks, HIV Pre and Post Test Counseling, Rapid HIV Testing, responding and assisting with emergencies, facilitating telehealth encounters, and various clerical and health-related duties associated with patient care, ADLs, and contributing to the safety, care, and comfort of patients.

Responsibilities:

  1. Facilitate Telehealth encounters as needed.
  2. Administer total care (bed bath, partial or complete).
  3. Assist with patient's hygiene - including skin care, mouth care, shaving, etc.
  4. Transfers patients - in and out of bed, stretcher, and wheelchair.
  5. Positions patients and carry out a range of motion exercise.
  6. Prepares patients for meals and assists with feeding as needed.
  7. Assists with unit patient admissions and transfers.
  8. Measures & records patients' vital signs (such as height, weight, temperature, blood pressure, pulse, respirations, etc.) in the electronic health record.
  9. Perform EKGs as required and follow outlined procedures for submitting abnormal EKGs.
  10. Perform dry sterile dressing changes and assist with other patient treatment.
  11. Administers hot and cold applications to patients, under supervision.
  12. Perform blood draws and urine testing.
  13. Chaperone staff in all clinical areas when requested.
  14. Perform tasks consistent with the PCA job description.

EMERGENCY RESPONSE

  1. Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, burns, and recovering post-operative patients.
  2. Provide clinically appropriate care as directed by the Registered Nurse and/or Physician.
  3. When clinically indicated, provide care, such as CPR, AED, prevent shock, control severe bleeding, prevent spinal damage, etc.
  4. Monitor patients' condition and keep them stable while en route to the clinic.
  5. Aid in preparing patients for transfer to the emergency units or hospitals.
  6. Document the medical treatment that is given to the patient.
  7. Keep the equipment clean after use; check and replace damaged and used supplies.
  8. Collaborate with the pharmacy to replenish supplies.

PHYSICAL DEMANDS

  1. Ability to lift fifty (50) pounds.
  2. Moving, lifting, or transferring patients may involve lifting up to 100 pounds.
  3. Ability to stand for extended periods.
  4. Fine motor skills.
  5. Visual acuity.






Qualifications

  1. A four-year high school diploma or equivalent (GED) required
  2. NYS Department of Health phlebotomy training program
  3. Patient Care Associate with EKG Certification
  4. AHA BLS Certification and Computer Competency required
  5. HIV Pre and Post Test Counseling training/certification

Hiring Salary: 46,975.50 annually

Scheduling note: PCAs are required to work every other weekend.

Schedule: 4pm - 12am, Sunday, Tuesday, Wednesday, Thursday, Saturday
