Job summary

Qualifications

• Required - High school diploma or equivalent.
• Preferred - 3 months of “personal care” experience in a home or clinical setting.
• Required - Basic Life Support (BLS) from the American Heart Association to be obtained within 30 days from hire and Crisis Prevention and Intervention (CPI) to be obtained within 30 days from hire.
• Must have computer skills and dexterity for data entry and retrieval of information.
• Effective verbal and written communication skills and the ability to present information clearly and professionally.
• Proficient with Windows-style applications and related software.
• Excellent organizational skills.
• Excellent time management skills.
• Ability to work a flexible schedule (e.g., 24/7, weekend, holiday, on‑call).

Responsibilities

• Supports the plan of care as delegated by staff nurse within scope of skills verification.
• Maintains a clean and safe environment in the patient care area.
• Provides care based on physical, psychosocial, educational, safety and related criteria for ages served.
• Communicates patient care needs with patients, families, and staff.
• Completes documentation of patient tasks.
• Adapts care to patient population and privacy requirements.
• Performs other related duties as assigned.
