Avenues Home Care of Tennessee is hiring a Patient Care Aide (PCA) in Knoxville to support elderly and vulnerable clients at home. You will assist with ADLs, medication reminders, meal prep, light housekeeping, transportation and companionship per each client’s plan of care.

Strong communication and reliable service are required; CNA preferred or equivalent caregiver experience. CPR/First Aid preferred. Flexible shifts include full-time, part-time, and PRN with training provided.

• Provide medication reminders, light housekeeping, laundry, meal preparation, shopping, and transportation.
• Provide companionship and report daily activities to supervisor using agency system.
• Adhere to client plans and communicate changes in condition to the team.
• Maintain confidentiality and document unusual events as requested.

Patient Care Aide - PCA

At Avenues Home Care of Tennessee, our caregivers are our front line and most common representative. As a Patient Care Aide - PCA, you will be caring for the elderly, vulnerable, disabled and most often those that cannot care for themselves. You are required to keep the client’s needs above all else and perform the following duties with the utmost care. Our office in Knoxville provides caregivers for the area including Savannah, Bearden, Kingsley Station, Halls, Farragut, and many others.

This program allows us to assess and understand our clients’ needs so that we can provide the best care possible. Our caregivers are trained in the Avenues Home Care Way to ensure they meet our high standards of service.

We take great care in matching the right caregiver to the right client. By using behavioral assessment tools, we gain insight into our caregivers and match that to the needs and desires of our clients.

Patient Care Aide - PCA Primary Responsibilities:

• Assist the client with Activities of Daily Living (ADLs) as specified in the client’s plan of care created during in-take. Those activities may include but are not limited to: bathing, dressing, grooming, other personal hygiene, toileting, transferring, eating and continence care.
• Assist with medication assistance, housekeeping services, laundry, meal preparation, feeding, shopping, transportation to and from appointments and companionship.
• Other household tasks as specified in the client’s plan of care
• To report daily activity to your supervisor through the agency management system and daily activity logs. Other communication avenues when available and as needed, including but not limited to phone calls, emails, agency app, face to face, etc.

As an Avenues Home Care of Tennessee Patient Care Aide - PCA, you will also:

• Practice safety in all activities concerning Avenues Home Care clients
• Be required to lift or push up to 50 pounds
• Demonstrate honesty, accountability and integrity
• Provide superior care to Avenues Home Care clients
• Remain vigilant of any potential care concerns or change in condition
• Keep your personal appearance in compliance with Avenues Home Care Policies, including abiding by company dress code
• Be timely in reporting for shifts or making scheduling changes
• Report any suspected abuse or neglect immediately up the proper chain of command, as required by law
• Document any behavior, events, conditions or people that are out of the ordinary
• Maintain confidentiality of records, correspondence, etc.
• Attend meetings and in-services as requested.
• Abide by all policies and procedures set forth by the Company.
• Ensure protection of confidential information that is stored, handled, or maintained by following Company policies and procedures related to confidentiality and/or HIPAA regulations.
• Other duties as assigned.

Patient Care Aide - PCA Required Knowledge:

• Work requires good communication skills.

Patient Care Aide - PCA Required Skills:

Must possess required skills and be able to explain and demonstrate, with or without reasonable accommodations, that the essential functions of the job can be performed. Caregivers are required to:

• Maintain an established work schedule.
• Effective interpersonal and communications skills including tact and diplomacy.
• Effective organizational and planning skills with attention to detail and follow through.
• Establish and maintain effective working relationships.
• Maintain confidentiality of work-related information and materials.
• Maintain “Fit for Duty” physical condition, a valid driver’s license, automobile insurance and will possess the following minimum requirements:
  • 18 Years of Age or Older
  • CNA License or alternate experience as a caregiver, preferred
  • CPR & First Aid Certifications, preferred
  • Reliable Transportation

Preferred Work Experience:

Prior experience assisting with ADLs or caring for elderly, disabled and other vulnerable clients
