SEEKING SPEECH THERAPISTS FOR HOME VISITS IN ALL AREAS OF DADE COUNTY
Summary & Objective

The Speech Language Pathologist is responsible is to provide direct and indirect speech language therapy services primarily on a visiting and intermittent basis in the patient’s home in accordance with the policies and procedures of Catholic Home Health Services of Broward.

Essential Functions
  • Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
  • Participate in the development and periodic review of the Plan of Care (POC).
  • Performs assessments and treats disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions.
  • Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
  • Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician.
  • Provide instruction and training to patients in use of alternative communication systems when appropriate.
  • Provide counsel and instruction to patients, families and healthcare staff.
  • Participate in care coordination activities and discharge planning.
  • Apply concepts of agency’s infection control plan and universal precaution in coordination/performing client cares activities to protect both patient and staff (OSHA).

Other Duties
  • Complete appropriate documentation in a timely manner to assure compliance with agency policy.
  • Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.
  • Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when recipient's right to access is clearly established and sharing of such information is in the best interest of the patient.
  • Maintain your required licenses, certificates and mandatory skill updates.
  • Comply with all policies, local, state and federal laws and regulations.
  • Provide other duties of healthcare team member
  • Perform other duties as assigned
Supervisory Responsibility
  • May serve as an interim department leader depending on need
Physical Requirements
  • Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.

Knowledge & Experience Requirements
  • Clear and Active Speech Language Pathology license to practice in the state of Florida.
  • CPR certification.
  • Bachelor’s or Master’s Degree in Speech-Language Pathology.
  • 2 years clinical experience preferred/
  • CCC required or currently completing CFY to obtain CCC.
  • 1-2 years of experience with a Medicare Certified Home Health agency preferred.
  • Knowledge of AHCA regulations, Medicare COP’s guidelines required.
  • Must have knowledge of computer office/clinical software.
  • Must be able to read, write and understand the English language.
