Hartford HealthCare is seeking a Hospice Spiritual Care Provider to deliver professional spiritual services to patients and families in Bridgeport. You will collaborate with the Hospice Interdisciplinary Team to assess and meet spiritual and bereavement needs, develop care plans, and document care in EPIC.

The role includes coordinating bereavement support, providing direct bereavement contact, and potentially on-call coverage during evenings, weekends, and holidays.

Qualifications

  • Bachelor’s degree with training in pastoral care.
  • One year experience in pastoral care, including experience in counseling the dying and the grieving.

Responsibilities

  • Provides professional spiritual services in accordance with hospice needs.
  • Collaborates with Hospice Interdisciplinary Team to assess and meet spiritual and bereavement needs.
  • Develops spiritual and bereavement care plans and maintains bereavement follow-up activities.
  • Coordinates bereavement care and support, including visits, phone calls, and group facilitation.
  • Provides care consistent with physician’s plan and documents in the medical record (EPIC).
  • May perform on-call spiritual care during evenings, weekends, and holidays.

Job description

Location Detail: 765 Fairfield Ave Bridgeport (10411)

Provides professional spiritual services in accordance with the needs of hospice patients, families, staff, and volunteers. Develops individual spiritual care plans, which demonstrate an effort to in collaboration with local clergy and spiritual counselors.

Collaborates with the Hospice Interdisciplinary Team in assessing and meeting patient/family’s spiritual and bereavement needs. Develops spiritual and bereavement care plans for each client receiving bereavement support.

Provides spiritual care consistent with the physician’s plan of care in clinical record. Includes travel time, visits with patients and families, and documentation.

Provides religious/spiritual consultation to clients and staff. Spiritual and bereavement care to patient/family at time of death. Coordinates bereavement follow up by Interdisciplinary Team Members and volunteers. Maintains a record of bereavement activities in EPIC.

Provides direct bereavement support to families through phone calls and visits. Is available to staff as well. Facilitate a bereavement support group.

Work with management, administrative, professional, paraprofessional, staff support, and area clergy. Adheres to standards of Agency’s quality assurance program, develops and maintains a resource group of community clergy to provide specific aspects of spiritual care. Will actively participate in the performance improvement process known as H3W. Participate in hospice support groups. Performs other duties as assigned.

May be required to do On-call spiritual care on evenings, weekends, and Holidays

Qualifications

Minimum Requirements:

  • Bachelor’s degree with training in pastoral care
  • One year experience in pastoral care, including experience in counseling the dying and the grieving.
Preferred Requirements
  • Masters level of Theology or Pastoral Counseling Clinical Pastoral Education units
  • More than one year of supporting the dying and grieving
