JWCH Institute, Inc + Wesley Health Centers is seeking a part-time Speech-Language Pathologist in Lancaster, CA to deliver skilled direct patient care to adult day health participants. You will conduct assessments, develop individualized care plans, provide therapy, and ensure thorough clinical documentation under supervision of the Program Director.

The role emphasizes interdisciplinary collaboration, regulatory compliance, and safety training.

Qualifications

• Graduate from an accredited Speech-Language Pathology program.
• Hold a current California license to practice as an SLP.
• Demonstrate strong assessment, treatment planning, and documentation skills.
• Experience with older adults or community-based settings preferred.
• All JWCH personnel are encouraged to be vaccinated against COVID-19.

Responsibilities

• Conduct initial assessments prior to enrollment with dated and signed records.
• Develop individualized care plans based on assessments.
• Perform reassessments and update care plans as needed.
• Complete documentation for each treatment session with dated signatures.
• Document quarterly progress notes per regulatory requirements.
• Discharge planning and summary documentation.
• Provide therapeutic activities and interventions aligned with the plan of care.
• Collaborate with the multidisciplinary healthcare team.
• Adhere to safety, infection control, and regulatory standards.
• Participate in required safety and compliance trainings.
