Job summary

UnityPoint Health in West Des Moines, IA is seeking a compassionate Registered Nurse (RN) for our Behavioral Health/Lakeview Counseling Center. This part-time specialty clinic role supports patient education, triage, and care coordination within a collaborative team.

Qualifications

Responsibilities

• Evaluate incoming patient calls and clinical concerns to assess symptoms and determine urgency.
• Provide telephone triage and guide patients to appropriate level of care.
• Coordinate care for patients with acute or complex behavioral health needs.
• Communicate concerns promptly to the clinical team.
• Educate patients and families on treatment plans and resources.
• Document patient interactions and care plans in the EHR.
• Coordinate follow-up to promote continuity and safety.

We are seeking a compassionate and motivated Registered Nurse (RN) to join our Behavioral Health/Counseling Specialty Clinic. Responsibilities

• Evaluate incoming patient calls and clinical concerns to assess symptoms, determine urgency, and identify appropriate next steps.
• Provide telephone triage and guide patients to the appropriate level of care or follow-up for behavioral health concerns.
• Coordinate care for patients with acute or complex behavioral health needs and communicate concerns promptly to the clinical team.
• Collaborate with counselors, behavioral health providers, and other healthcare professionals to support timely, coordinated patient care.
• Educate patients and families on treatment plans, medications, symptom management, and available behavioral health resources.
• Provide ongoing support and resources tailored to each patient’s individual needs and care plan.
• Accurately document patient interactions, assessments, interventions, and care plans in the electronic health record (EHR).
• Track patient needs and coordinate follow-up care to promote continuity, safety, and positive outcomes.

Qualifications

• Registered Nurse (RN) – Active/unencumbered RN license in state of practice or compact license including state of practice.
• All levels of RN experience will be considered. Prior experience in behavioral health, mental health, counseling, or specialty care is a plus, but not required.
• Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
• For positions within UnityPoint Clinic, team members may be required to work at or float to other locations to support optimized staffing.
