Job summary

St. Mary Center is seeking a Social Worker LSW/CSW for a part-time role (MWF 8am-1pm). We are a team-oriented facility focused on resident and staff well-being and growth.

Responsibilities include assisting residents with emotional adjustment, interviewing new residents, completing social services portions of MDS and care planning, and coordinating with interdisciplinary teams. Qualifications

• Bachelor's Degree in Social Work required.
• CSW and/or LSW certification required.
• 1-3 years of professional experience and/or training.

Responsibilities

• Assists residents with emotional adjustment to facility.
• Interviews new residents and records, social history, and assessments.
• Completes the social services portion of the MDS, care planning, baseline, etc.
• Refer to psychological services as needed.
• Schedules and attends interdisciplinary care plan meetings morning meetings, utilization review, etc.
• Writes summaries and progress notes as directed.
• Assists in discharge planning and post-discharge follow-up.
• Educates residents/representatives on Advanced Directives.
• Adheres to Social Services Regulations.

Responsibilities for a social worker

• Assists residents with emotional adjustment to facility.
• As directed, interview new residents and records, social history, and assessments.
• Completes the social services portion of the MDS, Care planning, baseline, etc.
• Refers to psychological services as needed.
• Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.
• Writes summaries and progress notes as directed.
• Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.
• Assists with discharge planning. Follows up after discharge.
• Conducts required Cut Letters
• Writes discharge summaries as assigned.
• Develops a professional relationship with each resident and family.
• Educates residents/ representatives on Advanced Directives.
• Adheres to all Social Services Regulations.

Qualifications for a social worker

• Minimum Bachelor's Degree in Social Work from an accredited College.
• Must obtain a CSW and/or LSW.
• One to Three years of professional experience and/or training; or equivalent combination of education and experience.
