Qualifications

• Must possess a current, unencumbered license to practice as a Social Worker in the State, or be permitted to serve in this role under federal and state rules.
• An associate degree in a human services field or related social services experience.
• Minimum 1 year experience in a human services field in a hospital, skilled nursing care facility, or other related medical facility.

Responsibilities

• Participate in planning, developing, organizing, implementing, evaluating and directing social service programs.
• Meet with administration, medical and nursing staff and other related departments in planning social services.
• Develop and maintain a good working rapport with intra-department personnel and outside community agencies to maintain social service programs.
• Assist in developing, administering, and coordinating department policies and procedures.
• Keep abreast of federal and state regulations and recommend changes to department policies.
• In the absence of the department director, serve on facility committees as appointed.
• Assist in developing policies for identifying medical and social needs of patients.
• Participate in community planning related to facility interests and patient needs.
• Participate in resident assessments, social care plans, and discharge planning.
• Interview patients/families in private settings and involve them in planning social services.
• Assist in arranging transportation to other facilities when necessary.
• Refer residents to appropriate social service agencies when needed.
• Provide information to residents/families regarding Medicare/Medicaid and financial assistance.
• Review complaints and recommend improvements; provide reports.
• Oversee the Social Services department and liaise with medical professionals.
• Create a warm, professional atmosphere and maintain confidentiality of resident information.
