Qualifications

• Active Speech-Language Pathologist licensure in Ohio or eligibility.
• Experience with older adults in post-acute care preferred.
• Ability to evaluate, create goals, and plan care.

Responsibilities

• Complete evaluations and re-evaluations; establish goals and plan of care
• Deliver skilled treatment and document progress per facility and payer standards
• Collaborate with the interdisciplinary team and communicate updates to patients/families
• Support discharge planning with clear recommendations and caregiver education
