Job summary

Tice Valley Post Acute in Walnut Creek, CA is seeking a Speech Language Pathologist (SLP) to evaluate and treat patients with communication, cognitive, and swallowing disorders. The role emphasizes compassionate, organized care and collaboration with an in-house rehabilitation team.

The ideal candidate holds an accredited SLP program degree, licensure or eligibility, and CCC-SLP preferred. Flexible part-time schedule with opportunities for professional development and a supportive team culture.

Qualifications

• Graduate of an accredited Speech Language Pathology program.
• Current state SLP license or eligibility to obtain licensure.
• CCC-SLP certification preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical findings.
• Provide speech, language, cognitive, voice, and swallowing therapy services.
• Monitor progress and modify treatment plans as appropriate.
• Maintain accurate, timely, and compliant documentation in accordance with facility, state, and federal guidelines.
• Collaborate with physicians, nursing staff, therapists, and other interdisciplinary team members to coordinate patient care.
• Educate patients and family members regarding treatment goals, progress, and home strategies.
• Participate in care conferences, discharge planning, and interdisciplinary meetings.
• Ensure compliance with all applicable state, federal, and company policies and procedures.
