Job summary

Elite Care Management, Inc. is seeking compassionate CNA/RN/LPN to join our private duty home health care team in Elburn, IL. You will work with one patient per shift, assisting with daily living activities and ensuring comfort.

Shift times include 7am–7pm or 7pm–7am on Saturdays; part-time hours with a pay range of $35–$40 per hour. Benefits include 401(k), health insurance, paid orientation, PTO, paid training, and vision insurance.

Qualifications

• CNA IL state certification required.
• Driver’s license required.
• TB test or chest x-ray within the last 12 months or QuantiFERON within the last 5 years.
• Current CPR card required.

Responsibilities

• Maintain records of patient care, condition, progress, or problems and report observations to supervisor or case manager.
• Support mobility by assisting patients with lifting and transferring as needed.
• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and eating.
• Light housekeeping duties, such as changing bed linens, laundry, cleaning, or preparing meals.
• Engage with patients to enhance quality of life through conversation and shared activities.
• Monitor patients’ pulse, temperature, and respiration.
