Job summary

UNIFI Healthcare is seeking a Licensed Field Nurse to provide patient-centered care in home settings across Richland, WA. This part-time role focuses on independence, safety, and quality of life for clients and families.

The role requires RN or LPN/LVN licensure, reliable transportation, and strong English communication. You will perform clinical tasks, document care, monitor vital signs, and collaborate with Case Managers to support individualized plans.

Qualifications

• RN or LPN/LVN license is required.
• Strong English communication skills are essential.
• Reliable personal vehicle and valid insurance are required.
• Clear criminal background check and negative pre-employment drug screening.

Responsibilities

• Monitor and record vital signs (heart rate, blood pressure, respiratory rate, temperature).
• Document intake and output.
• Administer and manage medications as prescribed.
• Assess client nursing needs on an ongoing basis.
• Provide nursing services in accordance with the established care plan.
• Promptly report changes in condition to the Case Manager and physician.
• Complete accurate and timely clinical documentation.
• Participate in development and implementation of individualized service plans.
• Observe and report mood, behavior, or emotional status changes.
• Communicate effectively with interdisciplinary team members.
• Perform light housekeeping to maintain a safe environment.
• Organize client living spaces to promote safety and comfort.
• Assist with errands and transportation, including medication pickup and grocery shopping.
• Daily travel to client homes and occasional visits to the agency office.
• Flexible scheduling, including potential weekends and holidays with compensation.
• Must be able to lift at least 25 pounds.

Job description

As a Licensed Field Nurse, you will provide comprehensive, patient-centered care tailored to each client’s individualized service plan. You will play a critical role in promoting independence, ensuring safety, and enhancing quality of life for our clients and their families.

Part Time 10 hours/week

• Provide compassionate personal care including bathing (bed bath, shower, or tub), dressing, grooming, oral care, and hygiene support
• Assist with mobility including ambulation, wheelchair use, and safe transfers
• Support toileting needs, including catheter and ostomy care
• Reposition bedridden clients and maintain clean, comfortable bedding
• Assist with meal preparation and support nutritional intake as needed

Clinical Responsibilities

• Organize client living spaces to promote safety and comfort
• Assist with errands and transportation, including medication pickup and grocery shopping

• Practice strictly within legal scope of licensure
• Maintain professional appearance and boundaries
• Promote teamwork, unity, and a culture of respect
• Adapt to additional responsibilities as assigned by leadership
• Daily travel to client homes and occasional visits to the agency office

• Must be able to lift at least 25 pounds

Minimum Qualifications

• Current, valid RN or LPN/LVN license
• Reliable personal vehicle with valid insurance
• Clear criminal background check and negative pre-employment drug screening
• Strong verbal and written English communication skills
