Godaelli Home Health in Northern Virginia seeks an experienced RN to support Personal Care and Respite Care programs on a part-time, 1099 basis. The role emphasizes assessment, care planning, supervision of aides, compliant documentation, and quality oversight in home settings.

You will coordinate with clients, families, and staff, ensure plans meet Medicaid waiver requirements, and maintain HIPAA privacy while delivering compassionate, professional home health services.

Qualifications

• Active, unrestricted RN license in Virginia or compact RN license.
• Minimum of 1 year of clinical RN experience.
• Experience with home care, personal care, respite care, Medicaid waiver services, or service facilitation preferred.
• Knowledge of Virginia Medicaid waiver programs, DMAS documentation, UAI assessments, Agency-Directed care, and Consumer-Directed care preferred.
• Strong assessment, documentation, communication, and organizational skills.
• Ability to work independently in the field and manage multiple client visits.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsabilités

• Conduct initial and ongoing in-home assessments for Personal Care and Respite Care clients.
• Develop, review, and update individualized Plans of Care.
• Provide required supervisory visits for Personal Care Aides and Respite Care staff.
• Support Consumer-Directed clients and Employers of Record with education and service coordination.
• Complete Medicaid, DMAS, waiver, and agency documentation accurately and on time.
• Support quality improvement activities and regulatory readiness.
