Job summary

Live Well Homecare is seeking a Care Management professional to guide discharge planning from admission through to discharge, coordinating with the healthcare team to ensure safe, effective patient throughput.

You will assess patients across all ages, develop care plans, coordinate transitions, and address complex psychosocial needs with strong communication and collaboration.

Qualifications

• Graduate of an accredited Nursing program with current state licensure.
• 2-3 years of acute care experience preferred.
• CPR certification at hire or within 90 days; case management certification preferred.
• Strong communication, assessment, time management, and critical thinking skills.
• Advanced computer proficiency and knowledge of care guidelines such as InterQual or Milliman preferred.
