Job summary

Options for Community Living, Inc. is seeking a part-time Residential Counselor for in-person work at Port Jefferson Station, NY. The role supports residents living with mental health diagnoses and requires weekend availability.

The ideal candidate will have a high school diploma, a valid driver’s license, a safe driving record, and access to a vehicle. Benefits include PTO, retirement plan with employer match, and ongoing training.

Qualifications

• Passion for assisting those diagnosed with mental illness. Must be a self-starter and well organized.
• Must be a high school graduate or the equivalent.
• Valid driver’s license, safe driving record, valid auto insurance and access to a vehicle is required.

Responsibilities

• Participate in planning and evaluation of individual service plans and monitor plan objectives.
• Write progress notes on service plan on a weekly basis.
• Train and assist residents in activities of daily living including household maintenance.
• Oversee medication maintenance and assist residents in developing self-medication skills as needed.
• Provide case management and follow-up service.
• Provide transportation to residents as needed.
• May attend team meetings and discharge meetings as needed.
• Provide household maintenance checks and repairs and conduct fire drills as needed.
• Notify Program Supervisor/Program Manager of any medical or psychiatric emergency.
