Oregon Integrated Health in Florence, OR is hiring a part-time Registered Nurse for our Rural Health Clinic. This patient-facing clinical role involves CCM, AWV, triage, VFC program support, and documentation in the EHR. Flexible scheduling within clinic hours and a supportive onboarding program with Lead RN mentorship are provided.

New graduates are welcome; prior ambulatory or primary care experience is a plus. Benefits include sick time, retirement matching, and continuing education support.

Qualifications

• Oregon RN license required.
• BLS certification required.
• Experience in ambulatory/primary care preferred.
• Proficient with EHR systems; training provided.

Responsabilités

• Chronic Care Management: manage an assigned CCM patient panel with monthly outreach and care plan updates.
• Medicare AWV: perform AWVs with health risk assessments and prevention plans.
• Clinical triage: provide telephone and in-clinic triage per protocols; route to appropriate care.
• VFC Program: maintain vaccines inventory, logs, counts, and compliance documentation.
• Document medical records in the EHR per clinic policy.
