Job summary

Avamere Skilled Advisors, LLC d/b/a Avamere Living seeks a Registered Dietitian for SNF Avamere King City (Finance) in Tigard, OR. This part-time, 16 hours/week role provides medical nutrition therapy to residents, aligning with goals and clinical status, and collaborating with Culinary Services.

You will conduct nutritional assessments, write care plans, review enteral/parenteral formulas, monitor outcomes, and participate in CQI and NAR meetings.

Qualifications

• Must hold active Registered Dietitian registration.
• CPR/BLS certification required.
• Dietitian license and/or certification to be obtained within 60 days from hire.

Responsibilities

• Complete comprehensive nutritional assessments and care plans for residents.
• Perform Nutrition-Focused Physical Examinations and nutrition interviews.
• Develop/evaluate enteral and parenteral nutrition prescriptions and adjust as needed.
• Monitor and document nutrition interventions and outcomes in resident records.
• Participate in Nutrition at Risk (NAR) meetings and interdisciplinary teams.
• Assist in facility menus review and substitutions as needed.
