• Associate degree in Physical Therapy from an accredited program.
• SNF therapy experience preferred.
• Strong communication and compassionate, patient-centered approach.

Responsibilities

• Provide individualized therapy services including screening, evaluation, treatment, program planning and implementation.
• Communicate patient progress, needs, and goals with residents, families, and care team.
• Complete documentation accurately and timely to support billing and payroll requirements.
• Support residents in regaining function, independence, and quality of life.
