Community Health Centers of America (CHCA) in California seeks a committed Physician Assistant (PA) or Family Nurse Practitioner (FNP) for a part-time role serving residents at Skilled Nursing Facilities in the Jackson area.

This position provides mission-driven care, three days per week, with responsibilities including patient evaluation, education, and thorough documentation, aligned with Medical and Nursing Board standards.

Qualifications

• Graduate of an accredited PA or FNP program with current licensure in California.
• Valid DEA registration and board certification.
• CPR/AED certification and proficiency with EHR systems.

Responsibilities

• Provide direct patient care including evaluation, diagnosis, treatment, and follow-up.
• Educate patients on diagnosis, treatment plans, and health maintenance.
• Maintain accurate, thorough medical documentation.
• Collaborate in quality assurance and clinical improvement initiatives.
• Work in accordance with CHCA policies and facility needs.
