Job summary

Infinite Medical P.C. is seeking a Primary Care Nurse Practitioner/Physician Assistant to provide on-site care for residents in long-term care facilities in Chandler, OK. This part-time role values meaningful patient relationships and collaborative care.

You will conduct exams, manage treatment plans, prescribe medications, and coordinate with facility staff. Requires an active Oklahoma NP/PA license and experience in long-term care; compensation is $90,000–$120,000 per year.

Qualifications

• Active and unrestricted NP/PA license in Oklahoma
• State-specific license in good standing in Oklahoma
• Minimum 6 months of experience in long-term care or skilled nursing facility
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person-centered healthcare

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
