Job summary

Infinite Medical P.C. is hiring a part-time Primary Care NP/PA to provide on-site care to residents in long-term care facilities in Littlefork, MN. This role emphasizes meaningful patient relationships and collaborative care in a dedicated team.

You will diagnose, manage chronic and acute conditions, perform exams, develop care plans, and educate residents and families. Minnesota license required; 6 months LTC experience preferred; flexible schedule and supportive environment.

Qualifications

• Active and unrestricted NP/PA/physician license.
• Minnesota license in good standing.
• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Strong communication, clinical, and critical thinking skills.
• Passion for elder care and person-centered healthcare.
• Prolonged standing/walking; lifting up to 20 lbs; computer work.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for residents
