Job summary

Qualifications

• Bachelor of Science in Physical Therapy from an APTA-approved school.
• Valid California Physical Therapist License.
• BLS (AHA) and ARC/Fire Card within 30 days of employment.
• Strong written and verbal communication skills.

Responsibilities

• Assess referred patients to determine medical condition and functional limits.
• Develop and administer a treatment plan with measurable goals.
• Provide inpatient and outpatient rehabilitative PT care, including modalities and gait training.
• Document progress notes and flow sheets per department and payor requirements.
• Prepare discharge assessment notes.
