Job summary

A Physical Therapist (PT) is needed on a part-time or PRN basis to support Independent/Assisted Living partnerships in St Peters, MO. The PT administers therapy under physician orders and supervises PT aides as assigned, in patient homes or facilities.

Responsibilities include patient assessments, evaluations, treatment planning, documentation, and interdisciplinary collaboration, with travel to patient locations as required.

Qualifications

• Bachelor of Science degree in Physical Therapy or equivalent.
• National Board certification and valid therapy license in Missouri.
• Current licensure in the state(s) practicing.

Responsibilities

• Assess patient functional ability and potential.
• Complete PT evaluation to determine PT benefits and establish a plan of care.
• Develop and implement medically prescribed PT plan of treatment.
• Document each visit accurately and timely.
• Instruct and supervise PTA; reassess progress at least every 30 days.
• Coordinate treatment with the interdisciplinary team.
• Communicate changes in patient condition to the team.
