Elderwood Skilled Nursing & Rehabilitation is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team within our skilled nursing and rehabilitation communities in New York. You will provide skilled PT evaluations and treatments, develop individual care plans, and collaborate with interdisciplinary teams to support resident progress.

Qualifikationen

• Graduation from an approved school of physical therapy.
• State license to practice physical therapy.
• Two years' experience in physical therapy preferred.
• Ability to direct others is necessary.
• English language proficiency at an intermediate or higher level.

• Provide skilled physical therapy evaluations and treatments per physician orders.
• Develop and update individualized care plans and therapy goals.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary clinical teams regarding resident progress.
• Participate in resident care planning meetings.
