Job summary

HealthPro Heritage in Columbus, OH is seeking a Physical Therapist (PT) for part-time outpatient care at a long-term care setting.

You will assess patients, design and implement therapy programs, document progress, and collaborate with a multidisciplinary team to restore function and improve quality of life.

Qualifications

• Must hold a Doctor of Physical Therapy degree and be licensed or eligible in Ohio.
• Excellent clinical assessment, treatment planning, and documentation skills.
• Strong communication and patient-centered approach.
• Ability to lift and assist patients as needed.

Responsibilities

• Assess patients' mobility, strength, and functional status.
• Develop and implement individualized therapy plans and exercises.
• Educate patients and families on exercises, posture, and home routines.
• Maintain accurate evaluations and progress notes; collaborate with the care team.
