Job summary

HealthPro Heritage is hiring a part-time or PRN Physical Therapist for our Port Jefferson, NY location. The role offers a flexible schedule, typically Monday through Friday, with no evenings or weekends, and a premium hourly rate of $55–$60.

You will assess patients, design and guide rehabilitation plans, document progress, and collaborate with a multidisciplinary team to deliver high-quality care in diverse settings within our communities.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills with a patient-centered approach.

Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, including mobility, strength, and flexibility.
• Treatment and Education: Provide therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain. Educate patients and families on exercises, posture, and body mechanics.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
• Collaboration: Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.
