Job summary

HealthPro Heritage is seeking a Part-time or PRN Physical Therapist for our assisted living community in Port Jefferson, NY. Work Monday through Friday with no evenings or weekends and enjoy a flexible schedule with premium hourly rates.

Responsibilities include patient assessments, therapeutic interventions, and education to improve mobility and function. Licensure in the state of New York is required or eligibility, with comprehensive documentation and teamwork to deliver high-quality care.

Qualifications

• Must hold a degree in Physical Therapy from an accredited program.
• Valid state licensure as a Physical Therapist or license-eligible.
• Strong clinical assessment and diagnostic abilities with patient-centered communication.

Responsibilities

• Assess patients’ physical condition including mobility, strength, and balance.
• Provide treatment, therapeutic exercises, and patient education to improve function and reduce pain.
• Maintain accurate documentation of evaluations, progress, and treatment plans.
• Collaborate with health care team to ensure cohesive patient care.
