Qualifications

Responsibilities

  • Evaluate residents per MD order and establish Treatment Plans and Goals that meet the specific needs of the individual.
  • Re-assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress.
  • Carry out treatment plan as indicated and supervise implementation by Licensed PTAs.
  • Provide thorough discharge planning that allows the patient to function at a maximal level of independence.
  • Educate staff, family, and caregivers as indicated.
  • Identify needs for program development and implement programs according to company policies and procedures.
  • Identify equipment needs of Physical Therapy Dept. and order equipment according to policy.
  • Complete all required documentation according to guidelines specified in Documentation Manual.
  • Coordinate in-services for facility staff to ensure safe carry over of programs.
  • Attend all facility required meetings.
  • Promote profession within facility and assist with coordinating events.
  • Attend Continuing Education courses specific to current position.
  • Maintain professionalism and follow regulations.
  • Supervise student affiliates as directed.
  • All other duties as assigned or directed.

ESSENTIAL FUNCTIONS:
  • Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.
  • Re-assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.
  • Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.
  • Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.
  • Educate staff, family, and care givers as indicated.
  • Identify needs for program development and implement programs according to company policies and procedures.
  • Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy.
  • Complete all required documentation according to guidelines specified in Documentation Manual.
  • Coordinate in-services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents.
  • Attend all facility required meetings.
  • Promote profession within facility, including assisting with coordinating special events i.e. National Physical Therapy Month.

  • Supervise student affiliates as directed.

QUALIFICATIONS:

PHYSICAL DEMANDS AND REQUIREMENTS
  • Ability to stand, walk, sit, stoop, bend, and reach for extended periods.
  • Ability to lift up to 35 pounds frequently (up to 66% of the time spent in performance of duties) and up to 50 pounds occasionally (up to 33% of time spent in performance of duties).
  • Manual dexterity that allows grasping, carrying, and holding equipment and supplies as needed to perform tasks.
  • Ability to see, hear, and verbally communicate with residents, visitors, and staff members during performance of duties.
