Qualifications

• Current and active California PT license.
• BLS certification from the American Heart Association.
• Ability to provide PT services within California licensure.
• Strong clinical evaluation and documentation skills.

Responsibilities

• Perform PT evaluations and provide evidence-based treatment.
• Develop individualized treatment plans based on findings.
• Provide therapy via physician referrals or direct access when appropriate.
• Communicate plans and progress to patients and healthcare team.
• Schedule and deliver PT evaluations and treatments as directed by leadership.
• Document assessments, plans, progress notes in approved systems.
