Job summary

PharMerica is seeking a licensed Pharmacist to join our long‑term care teams in Wyoming. You will evaluate medication orders for appropriateness, monitor patient data, and collaborate with physicians and facility staff to optimize therapy.

Required is a Bachelor’s Degree in Pharmacy (PharmD preferred) with LTC experience and applicable licensure. The role emphasizes accuracy, regulatory compliance, and clear communication.

Qualifications

  • Bachelor’s Degree in Pharmacy required.
  • One year of long‑term care experience.
  • Licenses/Certifications as required by state regulations.
  • Detail oriented, goal oriented, team player with strong oral communication skills.

Responsibilities

  • Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration; makes recommendations for therapy changes to physicians and facility staff as appropriate.
  • Maintains patient/resident database with current and past medical history, clinical and laboratory data, and concurrent medications.
  • Ensures appropriate laboratory tests for monitoring high‑risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborates with the healthcare team to provide clinical and technical support as needed.
  • Documents all communication with physicians, nurses, other staff and patients/residents in the medical record.
  • Implements policies for safe procurement, storage, distribution, and disposal of drugs; maintains necessary documentation.
  • Informs Pharmacy Manager of medication‑related errors and assists staff with related procedures.

Responsibilities

  • Maintains patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication (including over‑the‑counter and home remedies).
  • Ensures that appropriate laboratory tests for monitoring high‑risk conditions are ordered, evaluated, and discussed with appropriate members of the healthcare team.
  • Collaborates with the healthcare team to provide clinical and technical support as needed.
  • Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record.
  • Implements policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
  • Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and policy.
  • Informs the Pharmacy Manager of medication‑related errors.
  • Assists facility/agency staff with procedures related to safe and effective procurement, maintenance, administration, and disposal of drugs.
  • Performs other tasks as assigned.
