Job summary

Heart 'n Home Hospice, part of LHC Group / Optum, is seeking a Hospice Nurse Practitioner in Baker City, OR to provide compassionate palliative and end-of-life care. You will work with the Hospice Medical Director and a multidisciplinary team to deliver personalized care plans, manage symptoms, educate families, and document clinical findings.

The role emphasizes collaboration, comprehensive care coordination, and meaningful support for patients and families during advanced illness.

Qualifications

  • Current Nurse Practitioner certification in the state of practice.
  • National certification (ANCC, AANP, NBCHPN, or equivalent).
  • Current CPR certification.
  • 5+ years of nursing experience.
  • Valid driver's license, reliable transportation, and vehicle insurance.

About the Role Heart 'n Home Hospice is seeking a Hospice Nurse Practitioner to provide palliative care, symptom management, and supportive care for hospice patients. The NP works closely with the Hospice Medical Director, referring physicians, patients, families, and the interdisciplinary hospice team to ensure high-quality end-of-life care.

Key Responsibilities

  • Complete face-to-face hospice visits for qualifying patients
  • Assess patient condition and report findings to the Hospice Medical Director
  • Collaborate with physicians and interdisciplinary hospice team members
  • Develop, implement, and coordinate individualized plans of care

Compensation for this specialty generally ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
