Qualifications

• Two years minimum clinical or related experience.
• Certified Case Manager is preferred.

Responsibilities

• Identify individuals who meet program criteria for rehabilitation services.
• Promote and market rehabilitation programs.
• Perform admission of patients to services along the continuum of care.
