Job summary

Green Palms Health & Rehab at Crawford in Cleveland, OH is seeking a Part-Time or PRN Occupational Therapist to help residents achieve goals and improve quality of life within a supportive community.

You will design therapy plans, conduct assessments, collaborate with an interdisciplinary team, and document progress to inform treatment. Supervision for COTA may be provided as needed to ensure high-quality care.

Responsibilities

• Design and implement individualized therapy plans to help residents achieve their goals and overcome barriers
• Conduct assessments and evaluations to identify areas of strength and weakness, and develop strategies to address them
• Work collaboratively with our interdisciplinary team to address resident needs and promote a seamless care experience
• Document patient progress and outcomes to track progress and inform treatment plans
• Provide occupational therapy services to residents in a variety of settings, including individual therapy sessions, group activities, and resident-driven programs
• Provide supervision for Certified Occupational Therapist Assistants
• Develop and maintain effective relationships with residents, families, and caregivers to ensure a high level of satisfaction and engagement
• Stay current with best practices and evidence-based research in occupational therapy, and incorporate new knowledge and skills into practice
