Qualifications

• Graduated from an accredited Occupational Therapist program.
• Licensed and registered to practice in the state.
• Oversee and supervise OT Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation skills and writing ability.
• Conduct evaluations, progress notes, re-certifications and discharge documentation.
• Collaborate with disciplines to provide patient-centered care.
• Deliver exceptional patient service with clinical excellence.

Responsibilities

• Assess referred patients for rehabilitation needs and determine potential for rehab.
• Develop and administer treatment plans with goals aligned to patient capacity and tolerance.
• Set realistic goals, document patient condition, and educate patients and families.
• Ensure timely progression and discharge planning under Rehabilitation Director guidance.
• Maintain confidentiality and HIPAA compliance.
