Job summary

Creekside Center for Rehabilitation and Healing is seeking a part-time Occupational Therapist to join our dedicated team in the care of residents. The OT will assess patients, build rehab plans, and work under physician direction to maximize function and independence.

The role requires licensure, accredited OT education, and experience with long-term/geriatric care, with emphasis on documentation, collaboration, and patient-centered care.

Qualifications

• Must be graduate of an accredited OT program.
• Must hold a valid OT license and registration for the state of practice.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long-term/geriatric care preferred.
• Strong documentation writing skills.
• Conduct evaluations, weekly progress notes, re-certifications and discharge documentation.
• Collaborate with disciplines to provide patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Punctuality, professionalism, and adherence to schedule.
• Perform duties as directed by the Director of Rehabilitation.
• Comply with confidentiality, HIPAA, and Federal Residents Rights.

Responsibilities

• Assess referred patients and determine rehab needs.
• Develop and administer treatment programs with goals based on patient capacity.
• Set realistic goals and document patient condition and progress.
• Educate patients and families on therapy methods.
• Supervise Occupational Therapy Assistants.
• Collaborate with other disciplines to ensure best care.
• Maintain confidentiality and HIPAA compliance.
