Job summary

Continuum Therapy Partners seeks an Occupational Therapist in Monroe, WI for a part-time daytime role. The therapist will evaluate patients, develop individualized plans, and coordinate care to enhance functional independence. You will document progress, adjust treatments based on outcomes, and educate families on adaptive equipment and home safety.

Monroe WI location. Requires a Master’s or Doctorate in Occupational Therapy, a current state license, and NBCOT certification.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
