Continuum Therapy Partners in Long Prairie, MN is seeking a qualified Occupational Therapist to join our therapist-led team. The role focuses on evaluating and treating patients to improve functional independence and safety at home, under supervision of a leadership team that values clinical expertise.

Requirements include a Master's or Doctorate in OT, an active state license, and NBCOT certification. A minimum of one year of experience is preferred.

Qualifications

• Licensed Occupational Therapist in the state
• NBCOT certification required
• 1 year of experience preferred

Responsabilités

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
